Health Benefits Enrollment
Step 2: Make sure you have the following information available:
Your current personal email address and cell phone number.
Your legal name and the legal name(s) of your dependent(s).
Your social security number and the social security number(s) for your dependent(s).
The date(s) of birth of your dependent(s).
The names of your life insurance beneficiaries.
Additionally, you must submit the following information to the HR Department if it applies to your dependent(s).
Proof of dependent status - If you are adding a new dependent that is not a biological or step-child to a plan, you will need to provide official evidence such as a marriage certificate, birth certificate, qualified court order, etc...
Declaration of Informal Marriage Form H1057 (download here) - If your spouse has a different last name, you must provide this signed form to our Human Resources Department no later than June 26, 2020. If this documentation is not provided by this date, your spouse will be denied coverage and be removed as a plan participant.
For those with children who elect to take the Voluntary Critical Illness coverage, be sure to also enroll your children. There is no premium charge as it's included in the employee premium. You must enroll your children (up to the age of 26) during this open enrollment period to be eligible
Step 3: Log into Benefits Enrollment.

Your user name is your Employee Code as is appears on your new hire packet.
Your Employee Code begins with a J for JMEG and contains 5 digits beginning with a zero. For example: J01234
Your initial password is the last four digits of your SSN (Social Security Number).
